New Checklist to help people get the most from their Learning Disability Annual Health Check

The new Learning Disability Annual Health Check ‘Checklist’ for South Gloucestershire is here.

This Checklist is a tool for patients and their carers/supporters to fill in before attending a Learning Disability Annual Health Check. This Checklist supports people with learning disabilities and their carers to help recognise when things aren’t right, take actions to keep healthy, to speak up and seek help.

This Checklist will also be used to promote take-up of Annual Check-ups, raise awareness of their importance and contribute towards improving the health outcomes for those with Learning Disabilities. It will also raise awareness of Health Action Plans, highlighting factors that are sometimes missed (such as constipation) and the importance of health screenings (such as for breast and testicular cancer). 

The checklist has been created in collaboration with those with lived experience and with Bristol, North Somerset and South Gloucestershire Clinical Commissioning Group, Southern Brooks Social Prescribers and South Gloucestershire Council Public Health Team.
